Compounding Log (6400)

This log is compiled of formatted pages designed for recording entries of compounded prescriptions so that a reference is available and can be accessed quickly.

The log will assist staff in identifying the specific ingredients used, supplies, expiration date, lot numbers, and the quantity that was measured or weighed.  Spaces are also available for the signature of the person who prepared the formulation, the date of preparation, the assigned beyond-use date and the supplier.

Space is provided for chronological and current documentation of:  batches for anticipated dispensing, prescriptions dispensed from batches, and individual compounded prescriptions.

Opens flat for ease of use.

Pages are printed on bout sides.
